Gold Plan vision benefits include an eye exam every 24 months ($10 co-pay).
Materials such as frames ($25 co-pay), lenses, or contacts, ($25 co-pay) can be replaced every 24 months.
Platinum Plan vision benefits include an eye exam every 12 months ($10 co-pay).
Materials such as frames ($25 co-pay), lenses, or contacts ($25 co-pay) can be replaced every 24 months.
There is no waiting period to use your benefits. Benefits become effective on the first day of the month after signing up for the plan.
